Physical Examination

Athletes must have a current physical exam uploaded to this registration site. 

Once you have your physical completed by a doctor, you can scan it and submit the completed Physical Forms online on this registration. The physical is valid for 1 year from the date of the examination.

Additional Forms

If your son has food allergies, asthma, or epilepsy those forms must be uploaded in step number 4, this needs to be updated every year.

Immunization 

The immunization record must be uploaded in Step 4 of the online registration. Athletes without the required immunizations will not be cleared to participate in sports.

Payment of Fees

Fees are payable by Check, Cash, Venmo (@SJRHighschool1) or Credit/Debit Card

Please contact Ms.Mary Kay Buckley in the business office (contact information below) to make a card payment or set up a payment plan for athletic fees.

Baseball - Payment due when rosters are announced

Lacrosse- Payment due by 3/10

Golf - $475 due at registration, balance required when rosters announced

Developmental - $475

JV - $550 

Varsity - $650

Tennis - Payment due by 3/10
